The practice welcomes new patients from within the practice area.
During the pandemic, if you wish to register with the Surgery, please download the forms from our website. We are happy to accept scanned copies of the registration forms, together with :-
- photo diving licence or
- bus pass as proof of address
Proof of address:
- utilities bill
- lease agreement
- bank statement
The completed forms and ID can be returned via the following email address:
If you are unable to email the completed forms to us please send them in the post or, if necessary, bring them into the Surgery.
Once we have registered you, we may ask you to come into the practice for a new patient health check. Generally this would be if there is a delay in obtaining your records from your previous GP practice.
Although you will be allocated a GP, you can see any one of the eight GPs working at the Surgery.
Removing Patients from our list:
Sometimes, in very rare cases, it is necessary to remove patients from the surgery list because of their unacceptable behaviour or abuse of the service. If this happens, the patient will be notified in writing of the reasons behind the decision and who to contact to find out how to register with another GP practice. The Primary Care Support Services are able to advise on this and they can be contacted on 01392 449700.
We produce a regular newsletter for patients, which contains information on developments and current issues involving the surgery.
