The practice welcomes new patients from within the practice area.
If you wish to register with the practice, please come into the surgery to pick up the necessary registration documents. You will need to come into the surgery to collect the Registration Forms.
A health and medical history questionnaire should also be completed and we also ask for proof of address and photograph identification. You will be offered an appointment with the practice nurse for a new patient health check. This is important because it allows us to get to know you better and collect information on your past medical history. There can be a delay in obtaining your records from your previous GP practice and, therefore, the new patient information is vital to your new doctor.
If you find it difficult to relate to your registered doctor, you are able to see any one of the eight GPs working at the surgery.
Removing Patients from our list:
Sometimes, in very rare cases, it is necessary to remove patients from the surgery list because of their unacceptable behaviour or abuse of the service. If this happens, the patient will be notified in writing of the reasons behind the decision and who to contact to find out how to register with another GP practice. The Primary Care Support Services are able to advise on this and they can be contacted on 01392 449700.
We produce a regular newsletter for patients, which contains information on developments and current issues involving the surgery.
The newsletter is available from reception.
