Barton Surgery will be introducing Health Navigation to help patients get access to the right help, at the right time in the right place. One of the ways in which we can help release capacity in our Practice is to optimise our existing service and consider innovative ways of working.
Health Navigation involves signposting people to access appropriate services based on protocols. The patient is offered a choice to access the most appropriate service first which may not necessarily be the GP.
When you call or come into the practice for an appointment our trained staff will ask the nature of your call. This is not because they are prying it’s so they can help you get the right health professional for your needs.
